Prescriptions
Medication refills and new prescriptions as required
**Narcotics and controlled substances not refilled through this clinic** The exception to this is when a controlled substance is initiated by a healthcare member of the NPHWC. Our NPs only initiate controlled substance after careful assessment and consideration of the health needs of individual clients.
Test Results
Waiting on test results? We can provide you with completed test results. The NPs with NPHWC review all blood work, diagnostic imaging and consult reports that they initiated. The NPs of NPHWC record on your file when normal results have been reviewed and report to the receptionist anyone requiring a follow up on test results. If you wish to know if your test results have been reviewed, simply email or call us and our receptionist will check your file to determine if test results were reviewed.
For test results outside of NPHWC, we encourage you to follow up with the ordering clinician when you are awaiting blood work and/or diagnostic imaging results. However, in the event you are experiencing issues with accessibility to healthcare services and reports, you can book an appointment with NPHWC and we can access those test results if deemed necessary.

Complete referrals to specialists and other health care providers/services
Forms & Medicals
Various medical forms and assessments including pre-employment medicals, driver's medicals, health insurance medicals, and other medical/legal reports. Provision of notes for work or school due to illness after assessment by a NP. **Please note workers compensation forms cannot be completed at this clinic** You can email NPHWC or call to inquire if we are able to complete a specific form prior to booking an appointment. Fees are specific to the type of form or medical required.
Forms are taxable. Driver's Medicals are $125+ HST and all other forms are baseline assessment $55 and $25 per documented page + HST. Prior to completion of all forms (with the exception of driver's medical), the NP will be required to complete a comprehensive medical assessment to determine your physical and mental health issues which is our standard fee of $55 and may include blood work, imaging, etc.
Sexual Health and Reproduction
Screening, management, and couselling related to sexual health concerns.
Comprehensive Primary Care, Health Maintenance, Restoration, Secondary Prevention and Health Screening services
Complete a comprehensive health history and assessment
Formulate and communicate a diagnosis
Prescribe medications
Chronic disease management
